Required for All Chorus Families

Parents/Guardians of all RETURNING & NEW chorus members are required to complete the

online forms linked below. Choristers must be 18 or older to complete forms on their own.

Communication Guide

Click the link below to access our Communication Guide, which includes the multiple ways that   

Chorus staff and families communicate important information to one another.

Absence Request Form

  1. Known absences should be reported the Chorus staff at least 2 weeks in advance. 

  2. Please use this form, linked below, to also submit requests to arrive late and leave rehearsal early.

  3. After completing the form, please return it in-person or email it to Taylor Mills Logan at tlogan@clevelandorchestra.com

  4. If a chorister emails the form, at least one parent/guardian must be copied on the email.

  5. All absence requests must be signed by the chorister, parent/guardian, and director for approval.

  6. Taylor will contact the chorister and their family only if the excused absence request was denied.

Financial Aid Form

Click the link below to access the Financial Aid form. Beginning 23-24, COYC will be tuition free. You can still request financial aid to cover attire if needed.
